Incident Reporting

What is Incident Reporting?

 

Incident reporting is the documenting of incidents, and events, in the system that are either required by https://securityrisk.atlassian.net/wiki/spaces/SHD/pages/665845907 or to provide information for decision support.

Why do I need it?

Incident reporting provides a documentation for events of interest in the region. It allows:

  • Tracking of risk in your operational environment.

  • Auditing of operational tasks that you are contracted to support.

  • Reports for Customers to be created.

  • An easy way to provide information to your personnel and customers to help keep them safe and aware.

Creating Incident Reports (Events)

 

Web based Incident Report creation

An Incident Report is created on the https://securityrisk.atlassian.net/wiki/spaces/SHD/pages/682754084 or the https://securityrisk.atlassian.net/wiki/spaces/SHD/pages/682786881.

  1. Go to the Operational or Account View 

 

2. Select the Events tab

3. Click the Add button

  4. Fill in the mandatory information (marked with an '*')

 

5. Hit the save button!

 

You should now see your newly created Event in the list and be able to assign, edit or publish, distribute it.

Mobile Incident Report creation

Incident Reports can be created in https://securityrisk.atlassian.net/wiki/spaces/SHD/pages/682786888 and https://securityrisk.atlassian.net/wiki/spaces/SHD/pages/682885139.

  1. Open the newsfeed from the home tab

2. Select the '+' button

3. Fill in the mandatory information (in the required section)

Don’t forget to add a location by selecting Location Detail (https://securityrisk.atlassian.net/wiki/spaces/SHD/pages/690127020).

4. Hit upload draft to make it available to other personnel to assist in editing or submit for review.

Editing Incident Reports (Events)

An Incident Report is edited on the https://securityrisk.atlassian.net/wiki/spaces/SHD/pages/682754084 or the https://securityrisk.atlassian.net/wiki/spaces/SHD/pages/682786881 in the

  1. Go to the Operational or Account View

  2. Open the Events Tab

  3. Select the Event you wish to edit

 

4. Select the edit button on the detail panel

5. Change the fields you wish to update

 

6. Hit the save button!

Mobile Incident Report editing

  1. Open the newsfeed from the home tab

  2. Select the event you wish to edit (marked with the edit icon)

3. Select the edit button on the detail view

 

4. Change the fields you wish to update

5. Hit upload draft to make it available to other personnel to assist in editing or submit for review.

Operational Tasks

Events can be operationally managed from the Event Detail panel.

Users can:

  • Assign the report

  • Publish

  • Change distribution

  • Lodge a report

Assigning an Incident Report

Incident Reports and Events can be assigned to personnel to manage and update. Simply select the user’s name and hit the Assign button. The user will be notified that the event is assigned to them and can then work on completing the report.

Publishing

Publishing allows the report to be marked as finished and makes it available externally to your organisation based on the distribution.

Simply select Publish.

Sometimes personnel will request an incident report to be published. In this situation it is a simple case of Approving or Rejecting the request.

Distribution

Distribution states who is able to view the incident report or event on their https://securityrisk.atlassian.net/wiki/spaces/SHD/pages/665846089

A Business Client will see any report that is PUBLISHED and Distributed to Business Clients if:

  • Involved companies is blank; or

  • Their Business Client name matches the name of an involved company.

  • Public - visible to any user in the region including other companies personnel. Not all categories can be distributed to the public. This prevents confidential reports from being distributed externally. The following categories cannot be publicly distributed:

    • Site Incident

    • Security: Emergency Response

    • Security: Patrol Incident

    • Travel Incident

    • Asset Incident

    • Customer Incident

    • Occupational Incident

    • Continual Improvements

Business Clients and Public will only see the report if it has been Published.

Changing Distribution

From the event’s detail panel simply select the distribution you wish to change it to and hit Update.

Supervisors can only change distribution to Manager.

Lodging a Report

Some incidents may contain incorrect, inappropriate or copyright material.

Users can easily flag a report to identify the issue allowing the publisher or author to rectify the information by removing it from the external newsfeeds.

To retract a report simply change the distribution or update and re-publish the report.
